Getting Help From the GP

Appointments

You can book an appointment by:
  • Online with Klinik
  • Phone the surgery and ask reception to help you fill in the Klinik form

If you feel nervous or unsure, that’s completely normal. You can also request a chaperone to be present during any appointment.

 

Seeing the Doctor Alone

If you prefer to talk privately without a parent/guardian present, just let the reception team know.

Your Rights at the GP Surgery

As you turn 16, Vauxhall Practice recognises that you may not want to give your family the same permissions to your medical information as you used to. Following your 16th Birthday, we will send out a request for you to complete a form specifically stating who you want to access what information. You will be responsible for keeping this information correct and up to date and letting us know if you change your mind about consent at any time. You can find the form here if you have misssed our request or want to update your choices.
