Meet Tracy, our Place Based Care Coordinator

Helping older people and their families feel supported

Living with frailty can make everyday life more difficult. You may notice that you or someone close to you needs more help, or that arranging care feels increasingly complicated.

Tracy, our Place Based Care Coordinator (PBC), helps older people living with frailty and their families find their way through the support available. She works alongside patients, carers and care professionals to help bring the different parts of care together.

How Tracy can help

Tracy takes time to listen, understand what matters to you and explore where extra support could make a difference. Depending on your needs, she can help with:

  • Understanding your support needs: talking through how you are managing at home, any concerns you have and what you would like help with.

  • Coordinating care: helping communication between the professionals and services involved in your care.

  • Finding local support: connecting you with appropriate community services, practical help and opportunities to stay socially connected.

  • Supporting families and carers: listening to concerns and helping those who support you find information and support for themselves.

  • Navigating next steps: helping you understand who to contact and what happens next.

Your wishes come first

Everyone’s circumstances are different. Tracy’s approach starts with you: your priorities, preferences and the things that help you live well.

With your agreement, she can involve family members, friends or carers in conversations about your support.

Support for families and carers

Supporting someone living with frailty can be rewarding, but it can also feel worrying or overwhelming. You may be unsure what help is available or how to ask for it.

Tracy can help you explore these questions and find the right people to speak to. Your wellbeing matters too.
