Register with the surgery
We are pleased to welcome you to our practice. To help ensure your care continues smoothly during registration, please read the information below.
Important Advice About Your Medication
To avoid any break in your treatment:
- Please request an adequate supply of medication from your previous GP surgery before registering with us.
This ensures you do not run out while your medical records are being transferred. - Bring a current list of your medications (or a copy of your repeat prescription) when registering.
This allows us to record your medications accurately and helps prevent delays in future prescriptions.
Ordering Prescriptions With Us
Once you are fully registered:
- Prescriptions can be ordered:
- Via Klinik (our online consultation system)
- By using your nominated pharmacy
- By submitting a request at reception (where applicable)
- Please allow 5–7 working days for all repeat prescription requests.
This timeframe includes GP review and processing, so please order in good time.
Local Pharmacies
Commonly used local pharmacies include:
- Merricks Pharmacy
- Bulwark Pharmacy
- Boots Pharmacy
You can choose to nominate any pharmacy that is convenient for you.
Appointments and Online Access (Klinik)
- Most appointment requests are made using Klinik, our online consultation system.
- Klinik allows you to:
- Request appointments
- Ask medical or administrative questions
- Get advice or signposting to the most appropriate service

New Patient Health Checks
Newly registered patients may be offered a New Patient Health Check, depending on age and medical history.
This helps us understand your health needs and ensure your records are up to date.
Appointment Locations
Please note:
- Some appointments may take place at our Tutshill branch.
- Always check the location when booking or confirming your appointment to avoid confusion.
If you have any questions during registration, our reception team will be happy to help.
Catchment area
Before filling out a registration form, check you are in our catchment area.
To register
Either:
- download and complete a registration form
- complete the online GMS1 Registration
- visit reception and collect a paper registration form
Please also complete the new patient questionnaire by either:
- download and print the new patient questionnaire
- complete the online new patient questionnaire OR
- visit reception and collect a paper new patient questionnaire
If you complete registration online, you must come in to the surgery to sign your forms before being registered.
When you register with the practice you will be offered a new patient check. This will be an opportunity to discuss any health problems you may have. Your medical records may take some time to be transferred from your old practice.
In the case new babies being registered, please can you arrange to register your baby with the practice at least a week before attending for the first baby clinic.
Medical treatment is available from the date of registration. Please contact reception for further information.

Temporary Patients
If you are in the area for a short time and become unwell, we are able to register you as a temporary patient in order to treat you.
Please call the surgery on 01291 636100 and ask reception for more details.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.
